1. Mechanism of Contraction
Unlike skeletal muscle, smooth muscle does not use troponin. Instead, contraction is regulated by the phosphorylation of the myosin light chain.
- Calcium Entry: Membrane depolarization opens L-type voltage-gated
channels, increasing intracellular calcium.
- The Complex:
binds to Calmodulin, forming the
-calmodulin complex.
- Kinase Activation: This complex activates Myosin Light Chain Kinase (MLCK).
- Phosphorylation: MLCK phosphorylates the myosin light chain, allowing myosin to bind with actin $\rightarrow$ Contraction.
2. Mechanism of Relaxation (Vasodilation)
Relaxation occurs when myosin is dephosphorylated, often triggered by endothelial signals like Nitric Oxide.
- NO Production: Agonists (like ACh or Bradykinin) stimulate NO Synthase in endothelial cells to convert L-arginine into Nitric Oxide (NO).
- Diffusion: NO diffuses from the endothelium into the smooth muscle cell.
- cGMP Pathway: NO activates Guanylyl Cyclase, converting GTP to cGMP.
- Phosphatase Activation: cGMP promotes the activity of Myosin Light Chain Phosphatase (MLCP).
- Dephosphorylation: MLCP removes the phosphate from myosin, preventing actin-binding $\rightarrow$ Relaxation.

- Nitrates: Drugs like nitroglycerin provide an exogenous source of NO, leading to cGMP-mediated venodilation and reduced preload.
- PDE-5 Inhibitors: Sildenafil (Viagra) inhibits the breakdown of cGMP, prolonging the relaxation effect of NO in vascular smooth muscle.
- Dihydropyridines: Calcium channel blockers (e.g., Amlodipine) act on the L-type channels to prevent the initial calcium influx.
