Submucosal Resection (SMR) of the nasal septum is a traditional surgical procedure to correct a deviated nasal septum (DNS). Laser-assisted techniques have been introduced to modify or replace traditional methods for specific cases of turbinate or septal hypertrophy.

1. Clinical Application

  • Used primarily for nasal turbinate hypertrophy (Laser-Assisted Turbinoplasty) rather than formal septal SMR, which usually requires physical resection of cartilage/bone.
  • Lasers (commonly KTP, Diode, or Nd: YAG) are used to induce submucosal fibrosis and subsequent shrinkage of the soft tissue.

  • Cartilage preservation: Modern rhinologic surgery emphasizes preserving structural support (the “L-strut”) to prevent saddle nose deformity.
  • Laser Caution: Laser usage on the nasal septum must be extremely careful to avoid excessive thermal necrosis, which can lead to delayed perforation or cartilage collapse.
