Epidemic investigation is a systematic process. The goal is to identify the source and mode of spread to implement immediate control measures.

Steps in Investigation

  1. Verify the Diagnosis: Ensure it is an actual outbreak.
  2. Define the Case: Establish a clear “case definition” (Clinical/Lab criteria).
  3. Find Cases: Systematic search for more cases (Active surveillance).
  4. Descriptive Epidemiology: Map the cases by Time, Place, and Person.
  5. Generate Hypothesis: Based on the descriptive findings.
  6. Test Hypothesis: Perform an analytical study (usually Case-Control).
  7. Control Measures: Implement immediately (don’t wait for final results).
  8. Communicate Findings: Report the investigation to stakeholders.

  • Verification: The priority is always to verify the diagnosis and ensure the outbreak is real.
  • Epidemic Curve: The visual representation of cases by time of onset. A Point Source curve shows a single, sharp peak. A propagating curve shows a series of progressively larger waves.
  • Control Measures: These should be applied as soon as the source is suspected, even while the investigation is still ongoing.
  • Case Definition: Must be simple, objective, and applied uniformly to all potential cases.
