Infratemporal Fossa (ITF): Contents and Relations

The infratemporal fossa is a complex, wedge-shaped space located deep to the ramus of the mandible. It serves as a major hub for neurovascular structures transitioning from the cranial cavity to the face.

Primary Contents:

  • Muscles: Lateral and medial pterygoid muscles.
  • Arteries: Maxillary artery (and its branches, including the middle meningeal artery).
  • Nerves: Mandibular nerve (CN V3), chorda tympani (from CN VII), and the otic ganglion.
  • Veins: Pterygoid venous plexus.
Structure Key Functional Role
CN V3 Provides sensory to lower face/teeth and motor to muscles of mastication.
Middle Meningeal Artery Passes through the foramen spinosum; vulnerable in temporal skull fractures (epidural hematoma).
Pterygoid Plexus Extensive venous network; communicates with the cavernous sinus (potential pathway for infection).

The otic ganglion is functionally located in the ITF, suspended from the mandibular nerve. It serves as the relay for parasympathetic fibers (from CN IX) destined for the parotid gland.
