Breath-holding spells are benign, non-epileptic paroxysmal events occurring in children aged 6 months to 6 years, often triggered by emotional distress.


1. Classification

Type Key Characteristics
Cyanotic (Common) Triggered by anger or frustration, the child cries, followed by forced expiration and cyanosis.
Pallid Triggered by sudden pain or fright, the vagal response leads to immediate apnea, loss of consciousness, and pallor.

2. Clinical Approach

  • Diagnosis: Purely clinical, based on a detailed history of the event and the specific trigger.
  • Prognosis: Generally resolves spontaneously by age 5-6 years; reassure parents that they are not harmful.
  • Red Flags: Seizure-like activity not following the characteristic pattern or prolonged unconsciousness suggests epilepsy or other pathologies.
