Ventricular System and CSF

Understanding the flow of cerebrospinal fluid (CSF) and the anatomical narrow points is essential for diagnosing hydrocephalus and localizing intracranial obstructions.

1. The Flow Pathway

Lateral Ventricles → Foramen of Monro → 3rd Ventricle → Aqueduct of Sylvius → 4th Ventricle → Foramina of Luschka/Magendie → Subarachnoid Space

2. Anatomical “Choke Points.”

Structure Clinical Significance
Foramen of Monro Obstruction here causes unilateral hydrocephalus (if one side) or both lateral ventricles to enlarge.
Aqueduct of Sylvius The most common site for congenital obstruction, leading to enlarged lateral and 3rd ventricles.

3. CSF Dynamics

  • Production: Choroid plexus (located in the lateral, 3rd, and 4th ventricles).
  • Absorption: Arachnoid granulations (into the superior sagittal sinus).
  • Communicating vs. Non-communicating: Communicating hydrocephalus involves impaired resorption (e.g., meningitis/SAH); non-communicating is due to structural obstruction.

NEET PG Hint: When analyzing CT/MRI findings: if all ventricles are enlarged, think communicating hydrocephalus or atrophy. If only ventricles proximal to an obstruction are enlarged, it is non-communicating (obstructive). For instance, a posterior fossa tumor typically causes enlargement of the 3rd and lateral ventricles due to compression at the aqueduct.
