Venous Return (VR)

Venous return is the volume of blood flowing back to the heart per unit time. Under steady-state conditions, VR equals Cardiac Output (CO).

1. The Equation of VR

The determinants of venous return can be summarized by the formula:

VR = \frac{MSFP - RAP}{R_v}
  • MSFP (Mean Systemic Filling Pressure): The pressure in the entire systemic circulation when flow is zero. It reflects blood volume and venous tone.
  • RAP (Right Atrial Pressure): The downstream pressure impeding venous return.
  • R_v (Venous Resistance): The resistance to flow from the periphery to the right atrium.

2. Physiological Shifts

Intervention Effect on VR Mechanism
Increase Blood Volume Increase Increases MSFP.
Venoconstriction Increase Increases MSFP (decreases venous compliance).
Inspiration Increase Decreases intrathoracic pressure (lowers RAP).

Physiology Board Hint: For test questions, remember the venous return curve: the plot of VR vs. RAP. A decrease in MSFP (e.g., hemorrhage) shifts the curve downward; a change in blood volume shifts the intercept on the x-axis. Always link venoconstriction to increased VR via decreased venous compliance, not just blood volume.
