Learning Objectives

Distinguish between Perfusion-Limited and Diffusion-Limited gas exchange. Master Fick’s Law of Diffusion, understand the mechanism of Hypoxic Vasoconstriction, and recognize how specific lung diseases alter the DL_{CO} for the USMLE Step 1.


1. Perfusion vs. Diffusion Limitation

The rate of gas exchange is determined by how quickly a gas equilibrates between the alveolus (P_A) and the pulmonary capillary blood (P_a).

Type Mechanism Examples
Perfusion Limited Gas equilibrates early in the capillary. Exchange increases only if blood flow increases. O_2 (normal), CO_2, N_2O.
Diffusion Limited Gas does not equilibrate by the end of the capillary. The membrane limits exchange. CO, O_2 (in fibrosis or emphysema).


2. Fick’s Law of Diffusion

This equation explains how structural changes in the lung impact the movement of gases across the alveolar-capillary membrane. Formula:

3. Hypoxic Vasoconstriction

Pulmonary vessels constrict in response to low P_A O_2. Shunts blood to better-ventilated areas.

Exercise Increased cardiac output (perfusion). O_2 may become diffusion-limited as transit time decreases.

High-Yield Clinical Pearls:

  • The CO_2 vs O_2 Paradox: CO_2 diffuses 20 times faster than O_2. Therefore, patients with diffusion impairment (fibrosis) develop hypoxia (low O_2) long before they develop hypercapnia (high CO_2).
  • DLCO: Carbon monoxide (CO) is used to measure diffusion capacity because it is purely diffusion-limited. If DL_{CO} is low, the problem is with the alveolar-capillary membrane.
  • Exercise: In elite athletes or severe disease, O_2 exchange can shift from perfusion-limited to diffusion-limited because blood moves too fast through the capillary to fully equilibrate.
