Learning Objectives

Master the differentiation of Mood Disorders based on the severity and duration of emotional states. Learn to distinguish Bipolar I vs. II and the timing of psychotic features in Schizoaffective Disorder for the USMLE Step 1.


1. Spectrum of Mood States

Mood disorders involve an abnormal range of internal emotional states and a loss of control over them, causing significant distress and impairment in social and occupational functioning.

State Clinical Description
Mania Abnormally elevated mood/energy lasting >= 1 week (or any duration if hospitalized).
Hypomania Elevated mood lasting >= 4 days. No hospitalization, no psychosis, no severe impairment.
Euthymia The normal, baseline range of mood.
Dysthymia Persistent depressive disorder—mild/moderate low mood for >= 2 years.
Cyclothymia Fluctuating hypomanic and mild depressive symptoms for >= 2 years.

2. Differential Diagnosis: Mood & Psychosis

The presence and timing of psychotic features (hallucinations/delusions) are the highest-yield factor for distinguishing these disorders.

Diagnosis Required Mood Episode Psychosis Details
Major Depressive (MDE) >= 2 weeks of low mood/anhedonia. None (unless specified as “with psychotic features”).
MDE w/ Psychotic Features Full criteria for MDE met. Psychosis occurs ONLY during the MDE.
Bipolar I >= 1 Manic episode. Possible during manic or depressive episodes.
Bipolar II Hypomanic AND MDE. Possible during MDE; NEVER during hypomania.
Schizoaffective Concurrent mood & Schizophrenia. Psychosis MUST occur for > 2 weeks without mood symptoms.

High-Yield Clinical Pearls:

  • The 2-Week Rule: In Schizoaffective Disorder, there must be a period of psychosis (hallucinations/delusions) for at least 2 weeks while the mood is euthymic (normal).
  • Bipolar II Distinction: If a patient has psychotic features during their “high”, it is by definition Mania (Bipolar I), not Hypomania.
  • Episodic Psychosis: In primary mood disorders, psychotic features may be present at any time during mood episodes EXCEPT hypomania.
