Learning Objectives

Master the High-Yield Developmental Milestones from infancy through preschool. Categorize milestones into motor, social, and verbal/cognitive domains, and recognize the specific ages associated with key behaviors required for the USMLE Step 1.

1. Infancy (0–12 Months): “Parents Start Observing.”

The first year is characterized by the transition from primitive reflexes to intentional motor control and the beginning of social bonding.

Age Motor Milestones Social & Verbal/Cognitive
0–4 mo Lifts head prone (1 mo); Primitive reflexes disappear (Moro, rooting). Social smile (2 mo); Orients to voice (4 mo).
6–9 mo Sits alone (6 mo); Crawls (8 mo); Passes toys hand-to-hand. Stranger anxiety (6 mo); Separation anxiety (9 mo); Object permanence.

High-Yield Mnemonics & Tips:

  • The Rule of 3s: Tricycle at 3 yr; 300-1000 words at 3 yr; stacks 9 cubes (3×3).
  • The 4s: Hops on 4 (one) foot; plays with 4riends (cooperative play); tells detailed stories.
  • Clinical Red Flag: Failure to reach social smile by 2 mo or object permanence by 9 mo requires further evaluation for developmental delay.
