1. Embryonic Development & Pharyngeal Arches

The tongue develops from the pharyngeal arches, dictating its dual sensory and structural innervation.

  • Anterior 2/3: Formed by the 1st pharyngeal arch. General sensation is supplied by CN V3, while taste is mediated by CN VII.
  • Posterior 1/3: Formed by the 3rd and 4th pharyngeal arches. Both sensation and taste are supplied mainly by CN IX, with the extreme posterior portion innervated by CN X.

3. Motor Innervation & Muscle Actions

    • Genioglossus: Protrudes the tongue.
    • Styloglossus: Draws the sides of the tongue upward to create a trough for swallowing.
  • CN X (Vagus Nerve): Innervates the palatoglossus muscle, which elevates the posterior tongue during swallowing.

Clinical Notes & Pearls:

  • CN XII Injury: A lesion of the hypoglossal nerve results in tongue deviation toward the side of the lesion upon protrusion, due to the unopposed action of the opposite genioglossus.
  • Taste Pathway: All taste fibers converge on the nucleus tractus solitarius (NTS) in the medulla.
