The Thoracic Duct

The thoracic duct is the largest lymphatic vessel in the body, responsible for returning lymph from the entire body except the right upper quadrant (which is drained by the right lymphatic duct) to the venous circulation.

  • Origin: Begins at the cisterna chyli (typically at the L1–L2 vertebral level) in the abdomen.
  • Thoracic Entry: Enters the thorax through the aortic hiatus of the diaphragm (at the T12 level).
  • Mediastinal Path: Ascends in the posterior mediastinum, initially to the right of the midline, and crosses to the left side at the T4–T6 level (the transverse thoracic plane).
  • Termination: Empties into the venous system at the junction of the left internal jugular vein and the left subclavian vein.

Clinical Significance

Condition Mechanism
Chylothorax Injury or obstruction of the thoracic duct leading to leakage of lymph (chyle) into the pleural cavity.
Virchow’s Node An enlarged supraclavicular lymph node on the left side often signals malignancy (e.g., gastric cancer) that has metastasized via the lymphatic system.

The thoracic duct is intimately related to the esophagus and the azygos vein throughout its course in the posterior mediastinum. Surgical procedures in this area carry a risk of damaging the duct, which can be difficult to repair.
