Schuller View

The Schuller View (Lateral Mastoid Projection)

1. Clinical Definition

The Schuller view is a lateral oblique radiographic projection of the skull, specifically used to visualize the mastoid air cells and the petrous temporal bone.

2. High-Yield Indications

  • Mastoiditis: Evaluates the aeration and integrity of the mastoid air cells.
  • Temporal Bone Pathology: Assists in identifying erosion or sclerosis related to chronic ear infections (e.g., cholesteatoma).
  • Anatomical Landmarks: Useful for identifying the position of the temporomandibular joint (TMJ) and the external auditory canal.

3. Key Radiological Features

Feature Diagnostic Focus
Mastoid Cells Look for cloudiness or loss of bony septa, which indicates mastoiditis.
Petrous Ridge Evaluation of the superior margin of the petrous temporal bone.

4. Exam Pearls

  • Clinical Note: Similar to the Rhese and Water views, the Schuller view is largely of historical interest in clinical practice. High-resolution CT (HRCT) of the temporal bone is the definitive imaging modality for diagnosing mastoiditis, cholesteatoma, or middle ear pathology.
  • Board Exam Focus: If asked, connect the Schuller view primarily to the evaluation of the mastoid air cells.
