- Large Perforations: Whistling disappears. Instead, patients present with severe crusting, foul-smelling discharge, and recurrent epistaxis. The physical loss of the septum disrupts normal laminar airflow, causing high turbulence that dries out the adjacent mucosa, leading to persistent ulceration and crusting.
- Paradoxical Nasal Obstruction: Despite having a larger anatomical lumen, the loss of laminar airflow prevents normal mucosal sensation of air passage, causing patients to experience a subjective feeling of total nasal blockage.

A. Medical & Conservative Care (First-Line)

- Septal Buttons: Silastic or silicone prosthetic buttons can be inserted into the defect under local anesthesia to close the communication, instantly eliminating whistling and reducing crust formation.
B. Surgical Repair
Indicated for symptomatic patients who fail conservative management. Small-to-medium anterior defects can be repaired surgically, whereas large defects (greater than 2 cm) carry a high failure rate.
- Mucosal Flaps: Advancement or rotation flaps using local mucosa (e.g., inferior turbinate flaps or bipedicled mucosal advancement flaps from the nasal floor).
