High-Yield Otomycosis (Fungal Otitis Externa)

Otomycosis is a superficial fungal infection localized to the external auditory canal. It occurs secondary to a breakdown of the protective lipid-acid mantle of the ear canal, often triggered by hot, humid climates, frequent swimming, or the overzealous use of topical antibacterial ear drops.

Primary Pathogens & Pathognomonic Features

Thick white or greyish debris studded with innumerable fine black heads (conidiophores). Described as looking like “wet blotting paper” or a “wet newspaper” mass. Conidiophores ending in large spherical vesicles covered with sterigmata bearing black spores.
Aspergillus fumigatus Fungal mats present with a distinct pale blue or greenish-yellow tint. Club-shaped vesicles with single-rowed (uniseriate) sterigmata on the upper half.
Candida albicans Thick, white, creamy, or cheesy exudate coating the canal skin; may mimic a wet epithelial plug. Budding yeast cells displaying characteristic pseudohyphae on a KOH mount.

Clinical Triad

  • Intense Pruritus: Severe, unyielding itching within the canal is the primary hallmark and is far more pronounced than baseline pain.
  • Aural Fullness & Hearing Loss: Driven by mechanical accumulation of desquamated debris, cerumen, and fungal hyphae, creating a classic conductive hearing loss gap.
  • Watery Otorrhea: A thin, serous, or musty-smelling discharge tracking out of the canal.

Management Protocol

  1. Thorough Ear Toilet / Debridement: The absolute most critical step. Meticulous microscopic suctioning or dry mopping must be performed to strip the fungal mats. Topical antifungal drops cannot effectively penetrate un-debrided debris masses.
  2. Topical Antifungals:
    • Clotrimazole (1% drops): The broad-spectrum drug of choice, highly effective against both Aspergillus and Candida strains.
    • Nystatin: Effective against Candida, but exhibits poor coverage against Aspergillus species.
  3. Acidifying Agents: Application of 2% Acetic acid or aluminum acetate drops (Burrow’s solution) lowers the localized pH, creating a highly hostile environment for spore germination.
  4. Strict Dry Ear Precautions: Complete avoidance of moisture entry into the canal during bathing to halt fungal replication cycle dependencies.
