Caloric Reflex Test (Vestibular Evaluation)

The caloric test is a component of the electronystagmography (ENG) battery used to evaluate the **lateral (horizontal) semicircular canal** and its associated vestibular nerve branch ($CN\ VIII$). By introducing thermal gradients into the external ear canal, it isolates and tests each labyrinth independently.

Physiologic Mechanism

The patient is placed supine with the head elevated at **30 degrees**. This specific alignment positions the lateral semicircular canal perfectly vertical, allowing thermal induction to generate convective currents in the endolymph fluid:

  • Warm Water/Air (44^\circ\text{C}): Warms the endolymph closest to the tympanic membrane, causing it to expand and rise. This creates an ampullopetal (toward the ampulla) fluid current, increasing the firing rate of the vestibular nerve.
  • Cold Water/Air (30^\circ\text{C}): Cools the endolymph, causing it to sink. This creates an ampullofugal (away from the ampulla) current, depressing baseline neural firing.

The Classic Mnemonic: COWS

The direction of the **fast phase of nystagmus** is predicted by the pathognomonic COWS rule:

  • Canal Paresis (Vestibular Hypofunction): Calculated using Jongkees’ formula. A reduction in response on one side ($>20\text{–}25\%$) signifies a peripheral vestibular deficit. Common causes include **Vestibular Schwannoma (Acoustic Neuroma)**, Vestibular Neuritis, and Ménière’s disease.
  • Directional Preponderance: An asymmetric nystagmus response when comparing right-beating vs. left-beating vectors, indicating an uncompensated central or peripheral vestibular tone bias.
  • Calorics in Comatose Patients (Brain Death Exam): Used to evaluate brainstem circuitry (Vestibulo-Ocular Reflex / VOR):
    • *Intact Brainstem:* Cold irrigation causes slow tonic deviation of the eyes **toward** the irrigated ear (the fast corrective component from the cortex is lost).
    • *Brain Death:* **No eye movement** whatsoever across both warm and cold phases, indicating absence of brainstem reflex loops (pontine gaze center damage).
  • Contraindication Note: Standard water caloric tests must never be performed in patients with a **tympanic membrane perforation**, as it can trigger severe acute vertigo and middle ear contamination. Air calorics are utilized instead.
