M02.04.001 The olfactory nerve

The olfactory nerve, also known as Cranial Nerve I (CN I), is the first and shortest cranial nerve. It is a special visceral afferent nerve that transmits information related to the sense of smell.

Embryological Origin

• Derivation: The olfactory nerve is derived from the olfactory placode, a thickening of the ectodermal layer. The olfactory placode also gives rise to the glial cells that support the nerve.

Anatomical Course of the Olfactory Nerve

1. Nasal Epithelium

• Olfactory Receptors: Smell is detected by olfactory receptors located in the nasal epithelium.
• Fila Olfactoria: The axons of these receptors assemble into small bundles known as fila olfactoria (true olfactory nerves).
• Cribriform Plate: These bundles penetrate the small foramina in the cribriform plate of the ethmoid bone to enter the cranial cavity.

2. Olfactory Bulb

• Location: Once in the cranial cavity, the fibers enter the olfactory bulb, situated in the olfactory groove within the anterior cranial fossa.
• Mitral Cells: The olfactory bulb contains specialized neurons called mitral cells. The olfactory nerve fibers synapse with these mitral cells, forming synaptic glomeruli.
• Glomeruli: Second-order nerves emerge from these glomeruli and pass posteriorly into the olfactory tract.

3. Olfactory Tract

• Pathway: The olfactory tract travels posteriorly on the inferior surface of the frontal lobe.
• Division:
  • Lateral Stria: Carries axons to the primary olfactory cortex in the uncus of the temporal lobe.
  • Medial Stria: Carries axons across the medial plane of the anterior commissure to meet the opposite olfactory bulb.
• Connections: The primary olfactory cortex sends nerve fibers to other brain areas, including the piriform cortex, amygdala, olfactory tubercle, and secondary olfactory cortex. These regions are involved in memory and the appreciation of olfactory sensations.

Sensory Function

• Olfactory Mucosa: The sensory function of the olfactory nerve is mediated by the olfactory mucosa, located in the roof of the nasal cavity.
• Cell Types in the Olfactory Mucosa:
  • Basal Cells: Serve as stem cells for new olfactory cells.
  • Sustentacular Cells: Provide structural support, analogous to glial cells in the CNS.
  • Olfactory Receptor Cells: Bipolar neurons with dendritic processes projecting short cilia (olfactory hairs) into the mucous membrane, which detect odors.
  • Bowman’s Glands: Secrete mucus in the mucosa.

Clinical Relevance

Anosmia

• Definition: Anosmia is the absence of the sense of smell. It can be temporary, permanent, progressive, or congenital.
• Causes:
  • Temporary: Infections (e.g., meningitis) or local nasal disorders (e.g., common cold).
  • Permanent: Head injury, tumors in the olfactory groove (e.g., meningioma).
  • Neurodegenerative Conditions: Progressive anosmia may occur in Parkinson’s or Alzheimer’s disease.
  • Genetic Conditions: Anosmia is associated with Kallmann syndrome and Primary Ciliary Dyskinesia.

Olfactory Nerve Examination

• Procedure: The examination involves asking the patient about changes in taste or smell. Each nostril is tested separately with a specific smell (e.g., peppermint or coffee), with the patient’s eyes closed.
